Option 1 - Enrol In Person

Bring in your completed enrolment form or fill it in at CIS. Payment can be made by cash, cheque, money order, EFTPOS or credit card.

Option 2 - Enrol By Phone

Call CIS on 03 9347 9144 during office hours with your credit card details. One of our friendly office staff can assist you to complete your enrolment form.

Option 3 - Enrol By Mail

Send your completed enrolment form together with your payment (cheque, money order or credit card details) to:

Option 4 - Enrol By Fax

Fax your completed enrolment form credit card details to CIS on 03 9347 0175.
